Patients in Pocatello and the surrounding Bannock County area who carry Regence BlueShield of Idaho insurance are now facing higher out-of-pocket costs after Portneuf Health and Regence failed to reach a new contract agreement before a Sunday deadline.
As of June 15, Portneuf Medical Center facilities and its employed providers are no longer considered in-network for Regence BlueShield of Idaho members. The previous contract between the two organizations expired that day, leaving patients in limbo as both sides continue to negotiate.
What Broke Down in Negotiations
Portneuf Health and Regence have been in contract discussions for months without reaching a resolution. Portneuf has publicly accused Regence of routinely denying claims for care that physicians determined was medically necessary. The health system also contends that Regence owes it millions of dollars in unpaid claims — a dispute that has apparently become a central sticking point.
Portneuf’s position is blunt: “Significant issues remain unresolved.”
Regence, for its part, acknowledged that the prior long-standing contract has lapsed but pushed back on the idea that talks have stalled. In a statement, the insurer said, “Although Regence’s prior long-standing contract with the operators of Portneuf Medical Center has lapsed, negotiations continue at an accelerated pace.”
Whether that accelerated pace produces a new agreement in the near term remains to be seen. For now, Regence members who rely on Portneuf — the dominant health system in southeast Idaho — must navigate what is likely to be a costlier care experience until a deal is reached.
What Patients Need to Know Right Now
The out-of-network status does not mean Regence members are without any protection. Emergency services will continue to be covered at in-network benefit levels, which is a critical safeguard for patients who require urgent care at Portneuf’s facilities.
Additionally, some patients may qualify for continuity-of-care protections, which can allow individuals in the middle of ongoing treatment — such as those undergoing cancer care, pregnancy, or management of a chronic condition — to continue receiving services at in-network cost-sharing rates even when a provider leaves their insurer’s network. The specifics of who qualifies depend on individual plan details.
Regence is directing members with coverage questions or those seeking information about alternative providers to contact the insurer directly. Patients are encouraged not to delay in reaching out, particularly those with upcoming scheduled procedures or regular specialist visits at Portneuf facilities.
The practical impact of the contract lapse is significant for Bannock County residents. Portneuf Medical Center is the primary hospital serving Pocatello, Chubbuck, Inkom, and much of southeast Idaho. Regence BlueShield of Idaho is one of the state’s major commercial insurers, meaning the overlap of affected patients is substantial. Going out of network typically shifts a much larger share of medical costs onto patients — sometimes thousands of dollars per visit — unless or until the two sides reach a new agreement.
What Comes Next
Both Portneuf Health and Regence BlueShield of Idaho say they are still at the table, and Regence’s characterization of talks moving at an “accelerated pace” suggests some urgency on at least one side. However, the underlying disputes — including Portneuf’s allegations of claim denials and millions in unpaid balances — represent substantive financial disagreements that are not easily resolved overnight.
For Regence members, the best immediate step is to call the member services number on the back of their insurance card to understand exactly how their specific plan is affected, what out-of-network cost-sharing looks like, and whether they may qualify for continuity-of-care coverage. Those without urgent health needs may also want to ask Regence about in-network alternatives in the region.
